Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
This notice applies to TMS Boulder, a trade name of TMS Therapy Centers of Colorado.
Our responsibilities
We are required by law to maintain the privacy and security of your protected health information, to notify you following a breach of unsecured protected health information, to follow the duties and privacy practices described in this notice, and to give you a copy of it.
How we may use and disclose your information
Treatment. To provide, coordinate, and manage your care, including with other health care providers involved in your treatment.
Payment. To obtain prior authorization, verify benefits, bill your health plan, and collect payment.
Health care operations. For quality assessment, training, care coordination, business management, and similar activities.
As required or permitted by law. Including public health and safety activities, health oversight, judicial and administrative proceedings, law enforcement requests, serious threats to health or safety, workers' compensation, and research, in each case subject to legal requirements.
Uses and disclosures of psychotherapy notes, marketing communications, and any sale of protected health information require your written authorization. You may revoke an authorization in writing at any time.
Your rights
You have the right to inspect and get a copy of your health record; to ask us to correct information you believe is incorrect or incomplete; to request confidential communications by a particular method or location; to request restrictions on certain uses and disclosures, including a restriction on disclosures to a health plan for services you pay for in full yourself; to receive an accounting of certain disclosures; to get a paper copy of this notice; and to choose someone to act for you.
Complaints
You may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to this notice
We can change the terms of this notice, and the changes will apply to all information we hold. The revised notice will be available at our office and on this page.
